The goals of The Retreat are achieved through the development of an individualized care plan with the member and h/her primary and professional care partner(s) and include the following:

  • Delay, defer or eliminate the need for institutionalization
  • Provide respite for care partners
  • Assist with Activities of Daily Living (ADLs), such as ambulation, toileting, transferring, meals, nail and hair care
  • Provide therapeutic services for optimal recovery following hospitalization or rehabilitation stays
  • Maintain or improve the functional level of members
  • Provide nursing care for members to include, but not limited to, monitoring blood pressure, medications, weight, and diabetes and on an individual basis, injections, wound care, breathing therapy
  • Provide for growth and development of members through their engagement in arts, crafts, music, games, movement, and socialization
  • Prevent dehydration
  • Promote healthy nutrition
